When are forms needed:
Forms are required when a sport goes 'in-season'. For Cross Country, that is the first day of school (Wednesday, August 22).
The deadline for turning in forms for Cross Country will be Thursday August 30 (2nd week of school).
Forms are not needed for off-season practice, such as Cross Country summer practice (July, August) or off-season track (December, January, February).
But you can turn forms in to coaches at summer practices.
Clearance to compete and continue practicing with the team occurs when the forms have been reviewed by school administration to check on eligibility. This review can take several days after the forms are given to the cross country coaches. At the start of school, with so many sports starting at once, this review can take several days after the forms are turned into the coach.
It is recommended to complete forms for Cross Country during the summer and bring to summer practice or to practice on the first day of school.
Athletic Clearance Form must be completed for EACH sport.
It needs to be done for Cross Country. The student will also need to do a clearance form for any winter sport he participates in. And again for spring sports, such as Track and Field. Clearance form is not needed for off-season practice such as Cross country summer practice (July & August) or Off-season track (December-February).
Parent/student fill out and sign the information and insurance sections at the top, and the medical release at the bottom.
Leave Athletics and Health Office blank. Cross Country coach will route the Clearance form for their review and signature.
Athletic Packet must be completed once per year.
Usually for the first sport the student is in. If Cross Country is your first sport of the year, you need to do the Athletic Packet for Cross Country.
If a winter or spring sport at DP was your first sport of the year last year, then the athletic packet you did for that sport could still be valid for Cross Country.
In any case, the athletic trainer who keeps athletic packets on file must have a packet with a physical exam form that is less than a year (12-months) old for you to practice and compete in Cross Country.
Forms often are returned to students because something is wrong or missing.
The most common errors are:
1. Parent and/or student signature missing on the Clearance Form or on a page of the Athletic Packet.
Go over every page of the Athletic Packet carefully before taking to school to be sure the student and parent have signed everywhere they are asked to.
2. Missing doctor signature on the Health Form (physical exam page of the Athletic Packet).
3. Missing insurance information (company name; policy number) on the Clearance Form or first page of Athletic Packet.
What if you can't get an appointment with your family doctor in time
It is a firm requirement that a student needs to have a physical exam with results recorded, signed and dated by a doctor on the CIF Athletic Participation Health Form (page 5 of the Athletic Packet) to practice and compete in Cross Country (or any other team). If you can't get a timely appointment with your family doctor, we suggest going to a walk-in clinic (such as the Med Center near Bed Bath and Beyond in Goleta) to get the physical exam done.
Practice Locations and Times for Cross Country (page 2 of the Athletic Packet):
Cross Country practices both On-Campus and Off-Campus.
We practice at the track and on the trails and fields that are on campus.
We also do runs on the side-walks, roads, and trails of Goleta and Ellwood Mesa.
Cross Country practices on Monday, Tuesday, Wednesday, Thursday, and Friday.
Runners in cross country are expected on the weekend and on school holidays, but there is a rarely an organized practice with coaches on Saturdays or holidays.
Regarding meet days, check the Fall Schedule page at this website for the meet schedule. Some meets we take the whole team to are on Wednesday, some on Thursdays, and some on Fridays. Students will be excused from classes as needed to travel to these meets and will not return to school after the meet until the evening.
Cross Country practices both During School and After School.
Allow 2.5 hours for cross country practices.
Monday, Tuesday, Thursday, Friday, cross country practice starts during 6th period and continues after school.
Wednesdays (no 6th period), cross country practice starts after school and still goes for 2-2.5 hours.
Coaches will recommended that experienced runners run a few miles before school a few mornings a week, but this will not be expected of the whole team.

Driver Authorization form
Required for parents who want/need to be drivers for the team.
Needs to be accompanied with copy of drivers license, vehicle registration, and insurance coverages.
